Confidentiality

 

Your health records at the Orchard Surgery, and all the information held about you is confidential. We may share some of your information with other care providers in order to help plan and cost any treatment package for you.

Your information will not be passed on to third parties (insurance, solicitors etc) without your written consent.

If you have any concerns about confidentiality please talk to us straight away.

 


Data Protection

 

The Orchard Surgery is registered under the Data Protection Act 1998


As part of our Administration duties we need to send details, by computer link, to the West Sussex Shared Services Consortium. Please rest assured that NO medical information is sent with these details and ONLY personal details such as name, address and NHS numbers are sent - information which can be found on your medical card. If you have any questions about this please let us know.

 


Violent Patients


The practice supports the Government’s NHS Zero Tolerance Zone campaign. Violent or threatening patients will be reported to the Police and removed from the GP’s list. GP’s and staff have the right to care for others without fear of being threatened, attacked or abused

 


Freedom of Information Act 2000

 

If you require access to your patient record please let us know. If you have specific questions about small parts of your record this can usually be dealt with by your doctor, during a consultation. Some information may require you to complete an application form and there may be a fee involved.

The type of information that can be requested must be the type that we, or the authorities believe will not cause significant harm, In such cases, the information may be withheld under an appropriate exemption of the code of practice.
