right hand click and choose 'save target as', and then save it to your PC. Open it and complete the survey, and then you can return it as an attachment by clicking the email button at the bottom.
Complete on line
click on the link and the survey form will open up (takes a few moments depending on your computer speed). you can then select your answers straight away. When you have finished, just click on the email button at the bottom and follow the instructions.
Print and post - You can print the form out and return it to use by post
You can also save a copy of it to your PC for your records.
