  • Close liaison with community teams such as district nurses
  • GP home visits or telephone consultations for housebound elderly
  • Practice nurse visits for influenza vaccination, care plan preparation and long term condition reviews for housebound elderly patients
  • A Care coordinator (in place since 2015)

Orchard Surgery offers:

  • Effective recall for Chronic Disease Registers
  • 20-30 minute appointments as appropriate for reviews of conditions
  • LTC trained nurses - Stroke, CHD, Diabetes, COPD, Dementia
  • NHS Health Checks (by invitation) – early diagnosis
  • Insulin initiation
  • Community Diabetic nurse
  • Identification of Carers and offer of support where applicable

Families, Children & Young People

Orchard Surgery offers:

  • Flexible appointments before and after school
  • All acutely unwell patients seen on day
  • Focus on reassuring young people of confidentiality
  • Safeguarding Lead GP -  Dr Clare Stevens - collaborative working with all other agencies, prompt reporting and information sharing
  • Child protection training all staff, Levels 1-3 as appropriate
  • Contraception and FP services, sexual health sign-posting

  • All day telephone access— Doctors, advice, results

  • Child protection - Clinical system alerts, monitoring of missed appointments
  • Dedicated Safeguarding Administrator to ease coordinating/information sharing
  • Practice leaflets available in large print and easy read
  • Learning disability annual reviews
  • Adult Safeguarding Lead – Dr Clare Stevens – appropriate training for all staff

People with Mental Health Difficulties or Dementia

Orchard Surgery offers:

  • Full registers and care plans in place
  • Annual review in patient’s home if needed 
  • Robust recall system
  • GP or self referral to counselling/talking therapies (Time to Talk)
  • Carers lead, signposts and links to all available services
